How far is the Night Bazaar?

About 10-12km. A Grab taxi will cost around 150-200 THB and take 20 minutes.

Is breakfast included?

Usually no. It's an extra charge (~$5-7 USD) but features homemade yogurt, fresh fruit, and quality coffee.
